“Are My Shoes the Problem?” – A Footwear Q&A With an Orthopedic Doctor

By Dr. Milind A.Lokhande, [MBBS,MS Ortho], Joint Replacement and trauma surgeon

Patient:
“Doctor, I’ve been dealing with foot pain lately, especially after walking or standing for long hours. Could it be my shoes?”

Dr. Milind:
Absolutely—and you’re not alone. Footwear plays a crucial role in foot health and overall joint support. Wearing the wrong type of shoes can lead to foot pain, heel discomfort, knee strain, and even back issues over time.

Let’s walk through the most common footwear-related concerns I see in my orthopedic clinic.

Q: What kind of issues can bad shoes cause?

Dr.:
A lot more than you’d expect. The most common problems include:

  • Plantar fasciitis, which causes stabbing heel or arch pain
  • Flat feet complications leading to arch collapse and ankle pain
  • Metatarsalgia, or forefoot pain from tight, hard-soled shoes
  • Back pain from altered walking posture due to poor shoe design
  • Knee pain and hip strain caused by improper foot alignment

Supportive footwear isn’t just about comfort—it’s essential for joint and spinal health too.

Q: How do I know if my shoes are harming me?

Dr.:
If you regularly feel foot fatigue, pain in your heels or arches, or discomfort in your knees or lower back after wearing certain shoes—your footwear might be the problem.

Shoes that are completely flat, have no arch support, or are worn out can silently impact your alignment and walking pattern. And yes, flip-flops and high heels are two of the biggest offenders.

 Q: What makes a shoe “orthopedic-friendly”?

Dr.:
Orthopedic-friendly shoes are designed to support your body’s natural structure. Look for these key features:

  • Good arch support to reduce pressure on the plantar fascia
  • Cushioned soles to absorb shock, especially if you’re on hard surfaces
  • A firm heel cup to stabilize your ankle and prevent rolling
  • A roomy toe box to avoid crowding your toes
  • Durable outsoles that offer grip and balance during walking

If you’re struggling with chronic foot pain or have a medical condition like flat feet or plantar fasciitis, you may also benefit from custom orthotic insoles.

Q: Can expensive shoes guarantee foot comfort?

Dr.:
Not always. A high price tag doesn’t ensure the right fit or support. The best shoes for foot support are the ones that match your specific foot structure and activity level. Even affordable shoes can be perfect if they have the correct build and cushioning.

Try to shop later in the day when your feet are slightly swollen (a more accurate fit), and always walk around in them before deciding.

Q: How often should I replace my walking or running shoes?

Dr.:
If you walk or run regularly, change your shoes every 6–8 months or after covering 500 to 800 kilometers. Worn-out soles not only lose shock absorption but can change your gait—leading to new pain in your ankles, knees, or hips.

A Word from the Orthopedic Chair

Footwear is not a fashion afterthought—it’s a functional health tool. The wrong shoes can set off a chain reaction affecting everything from your foot mechanics to your posture and joint health.

If you’ve been struggling with persistent foot pain, heel discomfort, or knee and back pain, start by assessing your footwear. You may be surprised at how much relief you can gain just by choosing better shoes.
